Many of our participants are already reporting significant health benefits. If you are a Colne Valley PCN resident, please follow this link if you would like to find out more.

Our Resources

The educational resources that we have developed for this programme are free for anybody to use. We would encourage patients of the Colne Valley Primary Care Network to enlist the help of our experienced & enthusiastic health coach team to help you along the way. Just a reminder though, please ensure that if you suffer from any medical conditions or you are taking any long term medication, especially diabetes or blood pressure medication, you should speak to your doctor beforehand embarking on this sort of lifetyle change.
